We have been made aware of a need within our church family of those of us who may not have any family locally to contact should we have a medical or other emergency. We now have a downloadable form available for you to complete and file with the church office to alert us to whom you wish notified in such case. You can download it here or come by and pick up a form to fill out. All information will remain filed in confidentiality with the church office. This form can also be revised in the future.
